We will go through your form and fill it out at your intial consultation.

If you have chosen to skip straight to the First Kinesiology appointment without initial consultation, to save time at your first appointment, please download the following Client Health Form. It is helpful that I receive this in advance, though not absolutely necessary. You can either select ‘Fill & Sign’ on your pdf reader and fill it out electronically to email it to me, or print it to complete by hand and scan it to email back, or pop it in to Shop Holsitic prior to your appointment. If you are unable to do this, then just bring it along to your appointment.

If you would prefer me to complete it with you and go through it with you at length, please book an initial consultation, or let me know so I can extend your first appointment (this will be charged at the intial consultation + first kinesiology session rates and the appointment time extended to 3hrs).
